We directly support people with diverse and complex care needs in the community

Our Community Support Service includes complex care in the community and domiciliary care services.

Supporting and empowering adults and children with diverse and complex care needs to live life to the full and as independently as possible through:
  • short and long-term community care packages
  • placement breakdown support
  • transition care and support from a hospital to a community setting or from children to adult services
  • homecare services

Our services are tailored to support people with a range of complex care and support needs, including:

  • learning disabilities
  • mental health needs
  • autism
  • acquired brain injuries
  • behaviours that challenge
  • chronic conditions including dementia, eating disorders and addictions

Our comprehensive Homecare Services offer a full range of services to support people with their daily living activities including personal care, household help, hospital visits, medication support and companionship.
